When a PDF’s table of contents is wrong or gone, every object is found by reading the whole file, including those packed inside compressed streams.
Rescue PDFs that won’t open, or open with errors: broken internal tables, files cut short by a failed download or a full disk, damaged parts. Whatever survives is rebuilt into a clean file.

If even the list of pages is gone, a new one is made from every page that survived, in order. Damaged parts are left out rather than breaking the rest.
