From the headings
Headings are found by their size, as a reader would see them, and nested by it: the largest become chapters, the next sections. Headings set over two lines stay one bookmark.
Give a long PDF a table of contents readers can click through: build bookmarks from its headings in one go, then rename, nest and reorder them.

Rename any bookmark, move it up or down, nest it under another or bring it out a level. Its children move with it.
Bookmarks are written in this tab, and nothing is sent anywhere. The pages themselves aren’t touched.
